Hospital, Hospice & Senior Care Notary Services

When important documents need to be signed, traveling to a notary isn't always easy.

Forget-Me-Not Notary Services provides mobile notary appointments for patients, seniors, caregivers, and families in Wasilla, Palmer, and the Mat-Su Valley. I can meet you at a hospital, hospice facility, assisted living residence, or private home, subject to facility requirements and appointment availability.

My approach is patient, respectful, and unhurried. I explain the notarization process clearly, allow time for questions, and help make the appointment as comfortable as possible—because you deserve to understand before you sign.

Notary Services for Patients, Seniors & Families

Families often need important documents notarized during medical care, recovery, or transitions to assisted living. Forget-Me-Not Notary Services provides mobile appointments to help make that process more convenient and less stressful.

Common documents I can notarize include:

  • Powers of attorney (POA)

  • Advance healthcare directives

  • Financial and legal affidavits

  • Authorization and consent forms

  • Other documents requiring notarization

I work with patients, family members, caregivers, and care coordinators to arrange appointments at hospitals, hospice facilities, senior living communities, and private homes.

Every signer must be willing and able to communicate their decision and demonstrate awareness of what they are signing. I verify identity and complete the required notarial procedures, but I cannot provide legal advice or determine which documents someone needs.

What to Expect During a Hospital or Care Facility Notary Visit

I understand that hospital visits and care transitions can be stressful for patients and families. My goal is to make the notarization process as straightforward, respectful, and comfortable as possible.

Before the appointment:

  • Confirm the location: Provide the hospital, hospice facility, senior living community, or home address, including the room number if applicable.

  • Have the documents ready: The documents should be complete and ready for notarization, with any required signatures left unsigned until the appointment.

  • Need documents printed or scanned? Don't have access to a printer or scanner? I can help. Your mobile general notary appointment includes up to five printed pages at no additional charge. Just let me know when booking if you need documents printed or scanned. Any additional fees will be confirmed beforehand.

  • Prepare identification: Each signer should have an acceptable form of identification. Contact me beforehand if identification is unavailable.

  • Check with the facility: Confirm that visitors and outside notaries are permitted and whether any special arrangements are needed.

  • Discuss witness requirements: Some documents require witnesses in addition to notarization. Let me know in advance if witnesses are needed.

During the appointment:

I'll verify the signer's identity, confirm their willingness and awareness, and complete the appropriate notarial procedure. I allow time for questions about the notarization process without rushing the signer.

If a patient is sleeping, receiving medical care, unable to communicate their decision, or not ready to sign, we may need to pause or reschedule. I cannot proceed unless the applicable notarial requirements are met.

Not sure whether your loved one is ready for an appointment? Call or text (907) 671-0087 before booking. We can discuss the practical requirements and determine the next steps.

What Families Are Saying

Every family and every appointment is different. My goal is to provide a calm, respectful experience where people feel comfortable taking the time they need.

Here's how one client described his family's experience.

“Wonderful mobile notary! Cherissa was prompt, professional, and genuinely pleasant to work with. She introduced herself warmly to my aunt and uncle and was ready to get right to work, but she took their lead on the pace, never rushing them or making them feel hurried. She was organized, knowledgeable, and made a potentially tedious process feel easy. I'd happily use her again and highly recommend her to anyone who needs a notary who comes to you and treats your family with care.”
